Four Easy Ways to Register:

- Download and complete the Registration Form
(pdf) return it to the address on the form with full payment.
- Call Registration Services at 1(800) 538-2663 or (435) 797-0035. They will register you with a Visa,
MasterCard, Diners, Discover or AMEX Card Number.
- Fax your registration form with your credit card number. The 24-hour fax number is (435) 797-0636.

The totals shown must be received for each participant registered, regardless of credit status (i.e. awarded or audited).

If received after May 16, 2011.... $225

Credit Fee (requires 2 day registration):
2 semester credits ....$75
1 semester credit ....$45
School District Participant:
Districts not desiring to pay for the credit portion of the fee may pay the general registration fee shown above.
